There is no better time than now to raise awareness of the benefits Information Technology can bring to the US Health System. Adoption of technology is a driving force behind quality healthcare. Technology’s role in healthcare has expanded exponentially over the last 20 years. It has improved the way the healthcare industry delivers patient care and the way the public expects to be treated. VIZI Healthcare is one such quality technology provider proud to serve the healthcare community.
VIZI is a technology-enabled system-of-care that provides clinical, operational, and financial integration to delegated risk providers focused on enhancing medical home performance under alternative payment models. VIZI combines cloud-based technologies with remote and in-market care management experts to effectively coordinate and deliver community-based care for chronically ill senior populations. By providing timely access to care, and reducing the level of care variation & service utilization, VIZI helps seniors live independently in their homes while reducing healthcare expenditures.
Headquartered at Las Vegas, Nevada, VIZI Healthcare was founded in the year 2014. VIZI helps disparate care teams communicate and collaborate on a unified platform to better serve patients on their journey from the hospital back home. VIZI combines cloud-based technologies with remote and in-market clinical services to effectively deliver community-based care under financial partnership models. VIZI “smartly” integrates data sources, devices, and people across vendors and legacy IT systems.
Next-Gen Comprehensive Care for the People of Today
A smaller percentage of members of several health plans, managed care, and other organizations disproportionately spend more money on many avoidable medical events like hospital stays, emergency room visits etc. Proactively controlling these unnecessary costs is the key to performance-based healthcare. VIZI’s technology-based solution helps to reduce undesired costs by enabling smarter episodic transitional care to preferred networks and keeping members thriving in their homes & communities.
VIZI extends its support through its vIDN solution to connect disparate care teams serving the same patient so that they function as one coordinated team executing a unified care plan throughout the benefit year. This information and services gap closure ensures better quality care. In addition to that, VIZI’s technology-enabled services make sure appropriate and periodic care is provided to chronically ill seniors, thus eliminating unneeded hospital visits.
Furthermore, VIZI’s programs enhance CMS (Centers for Medicare & Medicaid Services) fee-for-service incentives while reducing administrative burdens for physician practices. By investing in provider networks and partnering with providers on a sub-capitated risk basis, VIZI enables clinical staff to focus on delivering great patient care and positive financial results.
VIZI’s Care for the Caregivers – Their Solutions
Neither the health plans nor the providers are armed with any technology solution to effectively integrate care while lowering costs. VIZI Healthcare’s vIDN (virtual Integrated Delivery Network) technology solution brings together people, processes and technology to build a smarter, more connected, health care experience for all stakeholders. The care system/platform increases visibility, accountability, and control across the entire care continuum without a heavy cost burden.
Data and connectivity issues often isolate providers and prevent them from collaborating across the community to meet the needs of their patients. But, with VIZI’s smart information technology, called VIZI Care Cloud, it becomes a worry of the past. The VIZI Care Cloud works across enterprise health IT infrastructures to deliver relevant patient information at the point of care and improving provider awareness and patient engagement in their care plan.
Because technology alone cannot fix healthcare in our rapidly evolving, value-based, environment, VIZI created Carepointe, a configurable care system that evaluates both clinical and non-clinical population needs, implementing a wide variety of remote and in-market care coordination services. It helps enhance existing care management infrastructures and improve the level of care, utilization, and member engagement gaps. The system has proven to reduce acute utilization, and improve cost and quality metrics for patients and health plan members.
Several studies prove that the cost and the quality of care vary heavily post the usual treatment. A broader post-acute care community with skilled nursing, inpatient rehab, home health, and other long-term acute care facilities, has different cost structures for different services. Such practices carry more financial waste, and quality of care deteriorates when members seek clinical or non-clinical care from the wrong providers. VIZI manages all aspects of post-acute care services under a sub-capitated risk relationship to provide financial control and clinical integration across Medicare and Medicaid populations. VIZI’s Carepointe Cap is a business solution for health plans and delegated risk physician groups struggling to control the cost and quality of managing post-acute care provider networks.
