Channels 360: Empowers Social Determinants of Health Networks
Channels 360 provide SDHNs with essential tools to engage community stakeholders with residents and connect people with available services. Integrate lists and directories of agencies into a single source. Enable non-connected agencies to manage referrals and close the loop. Intervene with at-risk patients using SDOH screening tools, connect them with available services and help them remove barriers to quality care. Patients are enrolled in Channels 360 programs based on HIE alerts, data from population health, analytics engines, and even spreadsheets. Programs are defined in Channels 360 using a simple graphical modeler to assign tasks to a coordinator. Routine tasks can be fully automated. Staff leadership can quickly set up and implement programs, including assigning staff and managing workload and performance through dashboards.

Michael leads the effort at Ready Computing, a passionate group of technologists exclusively focused on the creation, implementation, and management of innovative IT solutions. His expertise includes a thorough understanding of the healthcare IT standards, protocols, technologies, and architectures prescribed by the Office of the National Coordinator (ONC) and Centers for Medicare and Medicaid (CMS), Integrating the Healthcare Enterprise (IHE), the Health Information Technology Standards Panel (HITSP), Health Level 7 (HL7), the National Council for Prescription Drug Programs (NCPDP), and the Accredited Standards Committee (ASC).
Prior to starting Ready Computing in July 2011, Michael worked at InterSystems Corporation for nearly 16 years. As Senior Product Manager, Michael was not only responsible for the strategic direction, functional role, technical design, and overall adoption of HealthShare worldwide but also served as a senior developer that created core HealthShare components such as the IHE and CDA adapters. Michael was the Health Information Exchange “domain expert” within InterSystems and represented the company in key sales calls, marketing events, press interviews, technical workgroups, and other public activities. Michael also designed and executed the internal sales and technical training on HIE and HealthShare.
